What does hospice care look like?
Considering Hospice Care can be a daunting topic for everyone involved. At Pathways, our mission is to honor life by being compassionate and caring to those with advanced medical conditions, as well as those who are grieving. This can look different for everybody, and so we offer a number of different services to help at this critical time of life.
Pathways’ hospice services are available to anyone with a life expectancy prognosis of six months or less. The sooner you begin to use hospice care, the more support we can provide you. Below is more information about our services and what they could mean to you and your loved ones.
In Your Home
Many people prefer to remain in their own home for as long as possible. If you live in your own home, our team can bring our care and services to you. This way you can stay where you are most comfortable, and your loved ones can be nearby. Our on-call nursing care is available 24/7.
Assisted Living Care & Long Term Care
Are you already in an assisted living facility? We can also help care for you there. We have a team of nurses, specially trained volunteers, social workers, and music and art therapists that will work with those already serving you. You can never have too many people supporting you, and the extra care can provide you even more comfort.
Inpatient Care
Imagine a place that feels like home; comfortable, warm and inviting, filled with love and support. But this is no ordinary home. In this home, hospital-level care is available to relieve intense pain, manage symptoms, and provide compassionate, supportive care for you, your family member, or a friend.
Pathways Inpatient Care Center is a beautiful space specifically built for the purpose of providing you and your loved ones the highest level of care.
The Inpatient Care Center serves Larimer and Weld counties, providing patients and families a choice for intensive inpatient care when being at home is not possible when the patient’s condition meets acute hospice care guidelines. Each of the 12 rooms are large enough to accommodate groups of visitors and up to three family members who can choose to stay overnight. Visitors are welcome 24 hours a day, and even pets may visit with prior approval. Bedbound patients can enjoy the outdoors from the comfort of their bed.
Specialized Veteran Care
Larimer and Weld Counties together are home to approximately 38,000 military veterans. Pathways provides compassionate care for these veterans, as well as specialized support for their family members. We tailor our hospice, palliative, and support services to meet our veterans’ specific needs.
About Pathways
Pathways, located in Larimer and Weld county, is a local nonprofit providing symptom management and support for serious long-term illness (Palliative Care), expert medical and hospice care for individuals navigating the last months of life, and grief counseling and programs for those mourning the loss of a loved one or dealing with a tragedy. From our beginnings in 1978, when a group of nurses came together in Loveland to voluntarily care for a friend in the final months of her life, we’ve been caring for Northern Colorado residents regardless of financial circumstance.
We serve individuals living in Larimer County, western Weld County, and the Highway 85 and I-25 corridors including the towns of Fort Collins, Loveland, Greeley, and Windsor.
Our mission is to provide compassionate, excellent, comprehensive community care in Larimer and Weld Counties for those who have an advanced medical condition, serious illness and those who are grieving.
