Closed FoCo surgery center still seeking payments

FORT COLLINS – The Surgery Center of Fort Collins, which announced Aug. 12 that it was closing its doors because of “adverse business conditions,” is still working to collect on past due bills.

According to a news release issued Friday, “outstanding bills are due, and payments are still being accepted.”

Medical records held by the surgery center remain available. The center retained Cariend, a medical-records custodian, to manage the records for the next 25 years. However, Cariend is not a medical provider and does not handle billing, medications or referrals for the closed practice.

The center had been located at 1100 E. Prospect Road at Lemay Avenue.

According to the news release, “requestors should be prepared to complete the records release authorization form and provide a copy of their government-issued photo ID, as required by the HIPAA laws.” It said the fastest way to process requests for copies of medical records is to submit contact information at https://www.cariend.com/request-records/. An email would then be sent with instructions on how to securely upload authorization form and ID. Those without access to a computer or internet can reach Cariend by phone at 404-991-7080 but are reminded that HIPAA laws do not allow medical records to be released based on a phone or email request.

Completed authorization forms and government-issued photo ID also can be mailed to Cariend ROI, P.O. Box 1866, Thomasville, GA 31799-1866.
